Thank you for your recent request for the personal information that The PRP Lab holds on you. Firstly we hope you are completely satisfied with the aesthetic out come of the treatment you have received and look forward you seeing you in the near future.

As you may be aware from the 25th of May 2018 the General Data Protection Regulation (GDPR) came into effect and the law has changed regarding requests for your clinical records. This includes the photographs we took before and after treatment, your consent forms and medical history forms.

The change in the law also puts greater responsibility on us to protect your data and personal information and as such we at The PRP Lab have reviewed the process for ‘Subject Access Requests’.

At The PRP Lab your privacy and confidentiality has always been a priority for us. We treat hundreds of patients a month and feel the need to offer you all some reassurance that your data and images will not be released to anyone but you.

In order to ensure this in line with the General Data Protection Regulations we ask that you complete the request for information form (we can send this form to you via email). This needs to be signed and returned to us by recorded delivery. We also require that you include a certified copy of a 2 forms of ID.

Once this is received we will honour the request within the 1 month time frame set out in the GDPR guidance.
